At Aria Sonata Aesthetics, our goal is to provide a safe, professional, and welcoming experience for every client. The following policies help ensure clear expectations and a high standard of care.

By scheduling an appointment or receiving services, clients acknowledge that they have read and agree to the policies outlined below.


Age Requirement Policy


Aria Sonata Aesthetics provides elective cosmetic aesthetic services intended for adult clients.

Clients must be 21 years of age or older to receive aesthetic treatments or cosmetic procedures offered by the practice.

This includes, but is not limited to:

• neuromodulator injections (Botox, Dysport, Xeomin, etc.)

• dermal filler procedures

• microneedling treatments

• platelet-rich plasma (PRP) treatments

• chemical peels and advanced skincare services

Valid government-issued identification may be required to verify age.

Aria Sonata Aesthetics reserves the right to decline treatment if age requirements are not met.


Consent for Treatment


Prior to receiving any treatment, clients must provide informed consent acknowledging that they understand the nature of the procedure, potential risks, expected outcomes, and available alternatives.

Clients will have the opportunity to ask questions before treatment is performed.

Cosmetic procedures involve biological variability, and individual results may vary. No specific outcome can be guaranteed.

The treating provider reserves the right to decline or postpone treatment if it is determined that treatment is not appropriate or safe at the time of the appointment.

 

Good Faith Examination (GFE) Requirement

Certain aesthetic procedures offered by Aria Sonata Aesthetics are considered medical treatments and require a Good Faith Examination (GFE) prior to treatment.

A Good Faith Exam is a medical evaluation performed by a licensed medical provider to determine whether the requested treatment is appropriate and safe for the patient.

This evaluation may be conducted:

• in person

• through a secure Telehealth platform

• through a qualified third-party medical provider operating under the supervision of the practice’s Medical Director

The purpose of the Good Faith Exam is to evaluate the patient’s medical history, medications, allergies, and overall health status in order to determine whether the requested aesthetic treatment is medically appropriate.

The Good Faith Exam provided through Aria Sonata Aesthetics is offered at no cost to the client.

Medical clearance must be obtained and documented prior to performing injectable treatments or other procedures that require physician authorization.

Good Faith Exam Validity

A completed Good Faith Exam will remain valid for up to 365 days from the date of the evaluation, provided that there have been no significant changes in the client’s health status.

A new Good Faith Exam may be required before treatment if:

• more than 365 days have passed since the previous examination

• there has been a significant change in medical history or health status

• the client reports new medications, allergies, or medical conditions

• the treating provider determines that an updated evaluation is necessary for patient safety

Clients are responsible for informing the provider of any changes in their health, medications, or medical history prior to treatment.

Treatment Approval

Medical clearance obtained through a Good Faith Exam does not guarantee treatment will be performed.

The treating provider reserves the right to postpone or decline treatment if it is determined that the procedure is not appropriate or safe at the time of the appointment.

HIPAA Privacy Notice

Aria Sonata Aesthetics is committed to protecting patient privacy in accordance with federal and state healthcare privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA).

The Aria Sonata Aesthetics website hosted on Shopify is designed primarily for informational purposes, retail product purchases, and appointment scheduling access. This website is not intended for the collection or storage of protected health information (PHI).

Clients should avoid submitting medical histories, treatment information, photographs, or other health-related data through website contact forms or email.

When a client selects “Schedule Appointment”, they will be redirected to a secure scheduling and medical documentation platform used by the practice. This platform operates within a HIPAA-compliant environment designed to securely collect, store, and manage protected health information.

All medical records and treatment documentation are maintained within this secure patient system.

By scheduling an appointment, you acknowledge that you have read and agree to our Client Policies.


Appointment Cancellation & No-Show Policy

Appointment times are reserved specifically for each client.

To help ensure availability for all clients, we ask that you provide at least 24 hours notice if you need to cancel or reschedule an appointment.

Appointments canceled with less than 24 hours notice may result in a cancellation fee or forfeiture of any deposit associated with the appointment.

Clients who fail to attend their scheduled appointment without notice may be considered a no-show and may be subject to a no-show fee.

Repeated missed appointments may require prepayment for future bookings.

Late Arrivals

If you arrive late for your appointment, we will do our best to accommodate you within the remaining scheduled time. However, the appointment may need to be shortened or rescheduled in order to avoid delaying other clients.

Late arrivals may still be subject to cancellation policies.


Medical Photography & Video Consent

Medical photography and video documentation may be used for clinical purposes including:

• documenting treatment progress

• evaluating treatment outcomes

• maintaining accurate medical records

• assisting with treatment planning

Images captured for medical documentation become part of the client’s confidential medical record.

With separate written consent, photographs or videos may also be used for educational or marketing purposes including:

• website examples of treatment outcomes

• professional education

• social media content

• marketing materials

Clients may decline marketing use of their images without affecting their ability to receive treatment.

Before and after photographs represent individual patient outcomes and do not guarantee similar results for other clients.

**Required Clinical Photography

For medical documentation and treatment planning purposes, clinical photographs are required prior to and following certain procedures performed at Aria Sonata Aesthetics. These photographs allow the provider to accurately assess treatment areas, document baseline conditions, and monitor treatment progress and outcomes over time.

Clinical photographs taken for patient records are considered protected health information and are maintained securely within the patient’s confidential medical record in accordance with applicable privacy laws.

These images are used strictly for medical documentation and charting purposes and will not be used for social media, marketing, or promotional materials without the client’s separate written consent.

Clients may decline the use of their images for educational, marketing, or social media purposes; however, opting out of marketing use does not apply to photographs required for medical documentation.

Because clinical photography is necessary for proper treatment documentation and patient safety, clients who decline required medical photographs may not be eligible to proceed with treatment and the procedure may be canceled.

Treatment Eligibility

Even after medical clearance and consultation, the provider reserves the right to postpone or decline treatment if it is determined that treatment is not medically appropriate at the time of the appointment.

This may include circumstances such as:

• active skin infection or irritation

• pregnancy or breastfeeding

• certain medications or medical conditions

• unrealistic expectations regarding results

 

Agreement to Policies

By scheduling an appointment or receiving services from Aria Sonata Aesthetics, clients acknowledge that they have read, understand, and agree to these Client Policies.
